How to Get a Passport

The process of applying for a passport can be complicated. Review the requirements below and the resources from the U.S. Department of State to make sure you bring everything you need to your appointment.

Requirements

Completed Application

  • Fill out the DS-11 passport application. You can complete it in our office or on the U.S. Department of State website.
  • Bring the two pages that contain your information (do not bring instruction pages). Each page must be on a separate sheet. Black-and-white printing is acceptable.

Evidence of U.S. Citizenship

  • Provide one of the following: U.S. birth certificate; a previously issued U.S. passport (even if expired); Consular Report of Birth Abroad; or Certificate of Naturalization.

Proof of Identity

  • Bring a government-issued photo ID. The most common is a Utah Driver License or Utah ID.
  • If your ID is from another state, bring additional IDs (e.g., your out-of-state driver license plus a student ID).

Passport Photo

  • Bring a 2x2 inch passport photo, or have our staff take one for a $10 fee.

Payment

  • Fees payable to the U.S. Department of State (including the application fee and any expediting fees) must be by check or money order and stapled to your application.
  • If applying for multiple passports, use a separate check for each application.
  • Utah County fees (execution fee of $35 and photo fee) can be paid by cash, check, or Visa/Mastercard/Discover/American Express.

Parental Approval (for minors)

  • Under 16: both parents must accompany the applicant, or you must provide a notarized statement of consent from the absent parent, or evidence of sole legal custody (e.g., divorce decree or birth certificate showing only one name).
  • Ages 16-17: at least one parent must accompany.

Life or Death Emergencies

You may qualify for an appointment at a passport agency office (none in Utah) if you need to travel abroad in the next 2 weeks because your immediate family member outside of the United States has died, is dying or in hospice care, or has a life-threatening illness or injury.
