Application Summary
Some government offices outside of the United States may want data that is not printed on your marriage license/certificate, such as your date of birth, parents' names, or prior marriages, if any. This information appears on your marriage license application, which can be attached to the license/certificate as a second page.
Contact Information
Let us know who you are and how to contact you. The email address you provide will be primary communication for updates with your order.
Shipping Address
The shipping address is the mailing address where your documents will be sent. For addresses outside the United States, shipping is through FedEx International Economy. FedEx cannot deliver to P.O. Boxes or APO/FPO addresses.
For digital apostilles, the shipping address is replaced with the email addresses that will be receiving your digital documents.
Marriage License
Provide the first and last names of both individuals as they appeared on the marriage license PRIOR to that marriage. Provide the date of the marriage. If this matches a record in our system, you can proceed with your order. If you are unable to get a match, check to make sure you have entered the information correctly and are providing the correct names. If you still don't get a match, it is likely that Utah County was not the county that issued the license. You may be able to find the record through the Utah Office of Vital Records and Statistics. Contact our office if you have additional questions.
Upload Documents
Marriage Licenses/Certificates are vital records and can only be issued to someone with a “direct, tangible, and legitimate interest” in the record. We need to verify your identity before we can provide any marriage documents.
The ID image should clearly show your name, date of birth, and photo.
The “Selfie” photo should clearly show your face and you holding up your ID.
If you are ordering a document and you are NOT the one of the people married or the officiant, you will need upload evidence that you have a “direct, tangible, and legitimate interest” in the record. This could be a birth certificate showing a family relationship or a contract showing a legal/business arrangement.
Payment
You will see the itemized bill for your order with a link to pay by Visa, MasterCard, or Discover.
Timelines and Shipping
Apostille orders, including additional documents shipped with them, are sent in batches to the Lt. Governor’s office. Due to high volume, the process of issuing the apostille takes several weeks. FedEx tracking will only show activity once it leaves the Lt. Governor’s office. All other orders are shipped directly from our office to you. A FedEx tracking number will be provided for orders shipped internationally and orders paying for U.S. FedEx services. Orders shipped through the U.S. Postal Service have no tracking.
Order Tracking
After placing your order, you will receive an email with a link to the page for tracking your order. If you have an international address, apostille request, or error correction, part of the process is your verification of the information on your license/certificate. You will also do that at that link.
